Submission of this form by the party or parties listed above signifies the following statement: We agree to hold the organizers of this event and their agents harmless for all suits, claims or demands of every kind and character arising out of and in conjunction with this event. We hereby authorize the reproduction, sale, copyright, exhibition, broadcast and/or distribution of any event videotape or photographs without limitations. We understand the physical risks of entering dance competitions and social dancing and assume full responsibility for any injury or personal damages resulting from the event named above. We certify that we are 18 years of age or older (applicable to Legal Guardian if under 18 years of age).

Registrations must be posted by 7th October 2005
All competitors must be booked in at SAND BAY HOLIDAY VILLAGE for the weekend.

 Choreographers must submit their dance scripts with their competition application form

Couples / Pro-Am £4.00per dance / $6.00 US    Line Dance   £4.00 per dance / $6.00 US  
Teams, Large Troupes  £25 per Team / $35 US    Small Troupes (up to 4 persons)   £10.00 per Team / $12.00 US  
 ( Teams must be Pre-Registered to be legible for Prize Money. Forms must be Posted Before Closing Date.- (There will be no exceptions))
Choreographers  £5.00 per dance
